The area under the eyes plays a major role in how the face is perceived. Over time, loose skin, puffiness or prominent fat deposits under the eyes can create a tired or heavy look. Surgery on the lower eyelids can create a fresher, more rested and harmonious appearance, while preserving your natural features.
At Caolín, the treatment is always planned individually, with a focus on natural results, high surgical precision and respect for the balance of the face.
At a glance
At Caolín, lower eyelid surgery is most often combined with discreet fat grafting to the thinnest area under the eye - the so-called tear trough. This rebuilds lost volume and creates a smooth, natural transition from eyelid to cheek. It is a combination that markedly improves the overall result.
Lower eyelid surgery is a surgical procedure that treats bags, loose skin and prominent fat deposits in the lower eyelid. Depending on your starting point, the procedure can be performed in two ways – and the choice is made at the consultation based on your anatomy:
The incision is made on the inside of the eyelid, with no external scar at all. The fat deposits are removed or repositioned directly. This method is well suited when the skin is relatively firm and the main problem is prominent fat without excess skin.
A very fine incision is made along the lashes of the lower eyelid. This gives access to both fat and excess skin. The scar heals almost invisibly and is camouflaged in the skin's natural lines. The technique is well suited when there is loose skin.
Lower eyelid surgery is relevant when one or more of the following are present:
It is important to have realistic expectations. Lower eyelid surgery can reduce bags and tighten the skin markedly, but cannot remove all wrinkles or completely eliminate dark circles caused by pigmentation or thin skin.
Fat bags under the eyes that give a swollen or puffy look. This is often hereditary and can appear as early as your 30s.
Loose or wrinkled skin under the eyelid that contributes to an ageing look.
Dark circles partly caused by the shadow from prominent fat bags or lost volume in the tear trough.
A general look of tiredness that does not reflect how you actually feel.

Good to know
Here you will find the whole process step by step — from consultation to the first weeks at home. If there is anything you cannot find an answer to, bring it up at your consultation.
With the transconjunctival approach, the incision is made on the inside of the eyelid, with no external scar at all. With the external approach, a very fine incision is made along the lower lash line of the lower eyelid. This scar is camouflaged in the skin's natural lines and is almost invisible once healed.
Often, but it depends on the cause. Dark circles caused by the shadow from prominent fat bags are reduced markedly when the bags are treated. Dark circles caused by pigmentation, thin skin or blood vessels are not affected by the procedure alone; here, fat grafting to the tear trough or laser treatment can give a better overall result.
The tear trough is the hollow area that forms at the transition between the lower eyelid and the cheek. This hollow contributes to a tired or ageing look. By injecting the body's own fat into this area, volume is rebuilt, the shadow is reduced and the transition from eye to cheek becomes smoother and more youthful.
For the first few days, the eye area will be swollen, tender and discoloured. This is to be expected and subsides gradually. The swelling is the main reason why the final result is only fully visible after 3–6 months.
− Cold compresses for the first 48 hours relieve swelling and discomfort.
− Keep your head raised for the first few days – including at night.
− Socially presentable: Most people are ready after 10–14 days.
− Contact lenses: Avoid for 2 weeks after the surgery.
− Sport and strenuous activity: Avoid for 3–4 weeks.
− Sun on the scars: Avoid for at least 12 months – use at least SPF 50.
− Final result: Seen gradually over 3–6 months.
The result is long-lasting. Fat grafting to the tear trough gives permanent results, as the integrated fat behaves like the rest of the body's fat cells.
Yes, it is very common for both eyelids to be treated in the same session. It requires only one general anaesthetic and gives a complete, harmonious result across the whole eye area.
The surgery is performed under general anaesthetic. Afterwards, there is typically mild tenderness and tightness under the eyes for the first few days, which is managed with ordinary painkillers. Most people describe it as very manageable.
We recommend not wearing contact lenses for at least two weeks after the surgery. Glasses can be worn from the day after.
The result is long-lasting. Fat grafting to the tear trough is also permanent, as the integrated fat behaves like the rest of the body's fat cells. The skin will continue to age naturally.
Lower eyelid surgery is a well-documented procedure, but like all surgery it carries risks. The most relevant is ectropion, where the lower eyelid is pulled down and turns outwards, which can occur if too much skin is removed or if tightening scar tissue pulls on the lower eyelid. Other risks include bleeding, infection, temporary or permanent dry eye, asymmetry and, in rare cases, effects on vision. All risks are reviewed thoroughly at the consultation.
Every treatment journey at Caolín starts with a thorough, no-obligation consultation, where the surgeon assesses your anatomy and advises you on the best approach and on whether any complementary treatments could further enhance the result.
You are welcome to bring someone with you, and you can always contact us with questions along the way.
The process
The process starts with a thorough consultation, where you meet your plastic surgeon in calm, reassuring surroundings. Here we talk about your wishes, your expectations and the changes you would like to achieve.
The surgeon assesses the skin under your eyes, the elasticity of the tissue, any fat deposits and the overall proportions of your face to make sure the treatment is right for you. Together you will also go through the surgical process, expected healing and the things that are important to know before you make your decision. It is important to us that you feel well informed, at ease and met with both expertise and understanding throughout the entire process.
Once your treatment plan is in place, we guide you thoroughly through all the preparations leading up to the day of surgery. Among other things, we go through medication, supplements and other factors that can affect bleeding, healing and the final result.
Depending on the treatment, the surgery can be performed under local or general anaesthetic, and this is planned individually at the consultation. On the day of surgery, you will be welcomed by our team, and the surgeon carefully marks the area so that the treatment is tailored precisely to your anatomy and features.
During the surgery, the incision is placed discreetly, either just below the lash line or hidden on the inside of the eyelid, depending on your anatomy and treatment needs. Excess fatty tissue is removed or redistributed and, if necessary, the skin is gently tightened to create a smoother, more harmonious appearance.
The entire treatment is performed with great precision and respect for the natural anatomy of the eye. The goal is always a result that gives a more rested, fresh look without changing your natural facial expressions.
After the surgery, you can usually go home the same day. For the first few days, it is normal to experience swelling, tightness and bruising around the eyes, which is a natural part of the healing process.
We recommend that you take it easy, keep your head slightly raised and gently cool the area as instructed.
Your journey begins with a personal consultation. Here we go through your wishes and options, so that together we can create a treatment plan that feels right for you.
