A drooping or heavy brow is one of the changes that affects facial expression earliest and most noticeably. Low-set brows give the impression of excess skin on the upper eyelids, the eye area closes up, and the face can look tired, stern or tense, even when you are not. For some, it is not a question of age, but of anatomy. They have had low-set brows all their lives.
At Caolín, we offer two techniques: the endoscopic brow lift and the direct brow lift. The choice depends on your anatomy, your hairline and the result you want, and is always made in dialogue with the surgeon at your consultation.
At a glance
A brow lift repositions the brow and the surrounding skin to a more open and natural position. It opens up the eye area, lifts the brows and gives the face a fresher, more relaxed expression without changing your facial expressions or what makes your features unique. It can be done in different ways.
Both techniques give a lifted, more open look, but they differ in approach, incision placement and who they are best suited to.
The endoscopic brow lift is performed through very small incisions placed discreetly in the hairline. Using a camera instrument (endoscope), the surgeon works in the deep tissue layers. These are lifted and repositioned before being secured in a new, higher position. No incisions in the brows themselves or on the forehead, and no visible scars.
The technique is well suited to patients who want a discreet, natural lift without visible traces. It is minimally invasive, with a shorter recovery than the direct brow lift, and is frequently combined with the Caolín Signature Babylift, which works in the same region.
Advantages: No visible scars on the forehead/brows. Shorter recovery. Natural result.
Best suited to: Patients with good hair density and a good hairline, as the small scars can be hidden completely in the scalp.
Limitation: The surgery is not performed under local anaesthetic.
The direct brow lift is performed through an incision placed directly above the upper edge of the brow, at the transition between the brow hairs and the smooth forehead. The surgeon removes a strip of skin and tissue above the brow and lifts the brow to the desired position. The result is very precise and controllable, and the technique is particularly effective for pronounced brow drooping and asymmetry.
The scar is placed along the edge of the brow itself, where it is naturally camouflaged at the transition to the brow hairs. In most people it is almost invisible after healing, but placing the scar correctly requires experience and precision.
Advantages: Very precise and predictable result. Well suited to pronounced brow drooping and asymmetry. Can be performed under local anaesthetic.
Best suited to: Patients with significant drooping, thin hair or a high hairline.
Limitation: Leaves a scar along the edge of the brow, although it is effectively camouflaged where the brow hairs meet the skin.
A brow lift is relevant when the brow has descended to a level that affects the expression of the eye area or the overall balance of the face:
Gives the eyes a tired or downturned expression.
Closes up the eyes and can give a stern or tense expression.
May be caused by a drooping brow rather than the eyelid itself. In these cases, a brow lift can reduce the need for eyelid surgery.
Not a result of ageing, but of anatomical disposition.

Good to know
Here you will find the whole process step by step — from consultation to the first weeks at home. If there is anything you cannot find an answer to, bring it up at your consultation.
Yes. For milder dynamic asymmetry caused by uneven muscle movement, Botox can be a good way to relax overactive muscles and create good symmetry. For more pronounced brow drooping or asymmetry of the bone structure, a surgical brow lift permanently repositions the tissue and improves symmetry.
With the endoscopic brow lift, the incision is placed within the hairline, so there are no visible scars. With the direct brow lift, the scar is placed at the transition between the brow hairs and the smooth skin, where it can be seen but is naturally camouflaged.
In some cases, yes. A drooping brow contributes to excess skin on the upper eyelid, and a brow lift can reduce or eliminate the need for eyelid surgery. In other cases, both procedures are relevant and complement each other. At your consultation, the surgeon will assess what is right for you.
The endoscopic brow lift partly overlaps with the Babylift. The Babylift is Caolín's endoscopic upper facelift, which, in addition to the brows, lifts the outer corners of the eyes and the upper part of the cheeks. An isolated endoscopic brow lift focuses specifically on the position of the brow. The surgeon will advise you on which approach or combination is right for you.
A brow lift has few serious complications. They depend on the type of brow lift and are typically temporary. The most relevant risks are altered sensation in the forehead and hairline, asymmetry, visible scars and, in rare cases, hair loss along the scar.
The result develops gradually over 3-6 months. Many see a clear difference within the first few weeks, but the swelling subsides fully over the following months.
The result is long-lasting, but the face continues to age from the new starting point.
− Caolín Signature Babylift: Works in the same region and, in addition to the brows, lifts the outer corners of the eyes and the upper part of the cheeks. The endoscopic brow lift and the Babylift overlap technically and are combined or integrated at the surgeon's discretion.
− Upper eyelid surgery: A drooping brow contributes to excess skin on the upper eyelid. A brow lift and upper eyelid surgery complement each other and address the eye area as a whole.
− Facelift: For patients with changes across the whole face, a brow lift fits naturally into an overall facial rejuvenation plan.
− Fat grafting: Discreet fat grafting to the temples or under the eyebrow can complement the lift with natural volume and a soft, harmonious expression.
Yes, low-set or drooping brows affect men and women equally. At Caolín, the technique and placement are tailored to preserve a masculine expression, so the result always looks natural and proportionate.
Every treatment journey at Caolín starts with a thorough, no-obligation consultation, where the surgeon assesses your anatomy and advises you on the right technique: endoscopic or direct, and whether there are supplementary procedures that would further enhance the result.
You are welcome to bring someone with you, and you can always contact us with questions along the way.
The process
The process starts with a thorough consultation, where you meet your plastic surgeon in calm, reassuring surroundings. Here we talk about your wishes, your expectations and the changes you would like to achieve.
The surgeon assesses the anatomy of your forehead, the position of your eyebrows and the elasticity of your skin to find the technique that suits you best. Together you will also go through the surgical process, expected healing and the things that are important to know before you make your decision. It is important to us that you feel well informed, at ease and met with both expertise and understanding throughout the entire process.
Once your treatment plan is in place, we guide you thoroughly through all the preparations leading up to the day of surgery. You will receive advice on medication, supplements, smoking and fasting, among other things, as these factors can affect safety, anaesthesia and your body's ability to heal.
On the day of surgery, you will be welcomed by our team, and you will meet the nurse, the anaesthetist and the surgeon. Before the surgery, the plan is reviewed with you one last time, so that you feel calm, at ease and well prepared.
The surgery is performed under general anaesthetic and typically takes 1–2 hours, depending on the technique and any combined procedures. You go home the same day. With a direct brow lift, the surgery can be performed under local anaesthetic or sedation.
The surgeon lifts and repositions the tissue in the forehead and around the eyebrows to create a more open and harmonious expression. When the surgery is complete, the skin is closed carefully, with a focus on making the scars as discreet as possible. The area is protected with a dressing and compression to support healing.
After the surgery, you will be monitored by our staff until you are ready to go home. If you have had an endoscopic brow lift, you will have a bandage on your forehead for the first 10 days. If you have had a direct brow lift, you will only have surgical tape on the stitches.
In the days that follow, it is normal to experience swelling, tightness and slight discolouration in the area, which is a natural part of the healing process. We recommend that you take it easy, keep your head slightly raised and avoid activities that increase blood pressure and swelling in the early period.
Your journey begins with a personal consultation. Here we go through your wishes and options, so that together we can create a treatment plan that feels right for you.
