Motiva Preservé™ is a gentle, tissue-preserving breast augmentation technique that differs from the classic method in that the implant pocket is created without cutting through the breast's connective tissue and ligaments. Instead, the Motiva Preservé method uses a minimally invasive technique to separate the tissue gently, so the breast's natural support structures, muscle function and sensation are preserved as far as possible. The procedure can be performed under local anaesthetic and allows for mini scars and short downtime.
At Caolín, we offer Motiva Preservé® to you if you want more volume and a better shape, but with a procedure that is as gentle on the body as possible — with shorter downtime and a natural, soft result.

Motiva Preservé™ is well suited to you if you want a natural enlargement of your breasts while placing importance on a procedure that is as gentle as possible. The technique suits both those having a primary breast augmentation and those who want an augmentation combined with a slight lift.
The method is particularly relevant if you:
In a classic breast augmentation, the surgeon often cuts through muscle and connective tissue to make room for the implant. With Motiva Preservé™, a gradual, gentle technique is used instead, in which the tissue is separated rather than cut. This means less trauma to the surrounding tissue, less bleeding and typically faster healing.
The technique has been developed as part of a broader surgical philosophy of tissue preservation (Breast Tissue Preservation), where the aim is to preserve the breast's natural anatomy as far as possible — including the ligaments, muscle function and sensation in the nipple.
The area is numbed with a tumescent solution, which both anaesthetises the tissue and prepares it for gentle separation. This makes it possible to operate under local anaesthetic instead of general anaesthetic.
A small, discreet incision of 2.5–3 cm is made in the fold beneath the breast, where the scar will later be naturally concealed.
A blunt-tipped instrument is inserted through the incision and, using rotating movements, creates a tunnel to the implant pocket by separating the tissue rather than cutting it.
A balloon is inserted and gradually inflated, expanding the tissue in a controlled way and creating a precise, three-dimensional pocket tailored to your anatomy.
The implant is inserted through a protective insertion sleeve using a no-touch technique, which minimises the risk of infection and bacterial contact with the surface of the implant.
Motiva Preservé™ is used with SmoothSilk® Ergonomix2® implants (Motiva JOY), which are designed to move and feel like a natural part of the body.
The implants are soft and form-stable, keeping a round shape when you are lying down and taking on a more natural, teardrop-shaped silhouette when you are standing.
Available in three profiles — Mini, Demi and Full — depending on how much projection you want.
The surface and gel of the implants are designed to give a soft, natural feel with a low risk of inflammation and capsular contracture.
The final choice of size and profile is always made in dialogue with your surgeon at your consultation.

Good to know
Here you'll find the whole process step by step — from your consultation to the first weeks at home. If there's anything you can't find an answer to, bring it along to your consultation.
With Motiva Preservé, the implant is placed over the pectoral muscle (prepectorally), without the muscle being cut or detached. This means less trauma, less pain after the surgery and preserved muscle function, while the implant is still well camouflaged by the natural breast tissue.
The incision is made in the fold beneath the breast as a small, discreet incision of 2.5–3 cm, which is naturally concealed in the inframammary fold and becomes almost invisible over time.
Motiva Preservé™ is a well-documented and gentle procedure, but as with all surgery, there is always some risk. The most relevant complications are capsular contracture, bleeding and infection, but the tissue-preserving technique and the no-touch insertion of the implant have been developed precisely to reduce these risks compared with traditional techniques. All risks are discussed thoroughly at your consultation.
Every treatment journey at Caolín begins with a no-obligation consultation, where we assess whether Motiva Preservé™ is the right solution for you. You are welcome to bring someone with you, and you can always contact us with questions while you are considering your options.
In a classic breast augmentation, the tissue is cut to make room for the implant. With Motiva Preservé™, the tissue is instead gently separated without being cut, which typically means less trauma, less swelling and a shorter recovery.
No, the technique has been developed so that it can be performed under local anaesthetic with a tumescent solution. If you would prefer a general anaesthetic, however, that is still possible.
Most women can breastfeed normally after the procedure. The technique has been developed precisely to preserve glandular breast tissue and sensation as far as possible.
Modern quality implants can be expected to last at least 15-20 years and often considerably longer, but a replacement will probably become relevant at some point.
Yes, in many cases the technique can be combined with a lift. Whether this is relevant for you is assessed at your consultation.
Yes. Mammograms can still be performed after Motiva Preservé™ surgery. Always remember to tell the radiography staff about your implants.
The process
The process begins with a thorough consultation, where together we assess whether Motiva Preservé™ is the right solution for you. We go through your anatomy, the quality of your skin and your wishes regarding size and shape, and you have the opportunity to try different implant sizes, so you get a realistic sense of the expected result.
There is a cooling-off period between the consultation and the surgery, so you have plenty of time to think the decision through.
Once your treatment plan is in place, we guide you through the preparations leading up to the day of surgery. You will receive information about fasting, medication, supplements and smoking, as these factors matter for both safety and healing.
• Blood-thinning medication is typically paused 7 days before the surgery
• Arthritis medication is typically paused 5 days before the surgery
• Herbal remedies such as ginseng, Q10 and fish oil are typically paused 2-3 weeks before the surgery
As Motiva Preservé™ is usually performed under local anaesthetic, you are awake for most of the procedure, but without pain in the area being operated on. The surgeon first numbs the tissue with a tumescent solution, after which a small incision is made in the inframammary fold.
Using the specially designed instruments, the implant pocket is created gently, and the implant is inserted through a protective sleeve. The procedure typically takes 45–60 minutes, and the wounds are finally closed with dissolvable stitches. (It is possible to choose a general anaesthetic if you prefer.)
Because the technique is gentler on the tissue, many patients experience less swelling, tenderness and downtime than with a classic breast augmentation. Most people can return to light activities within 0–2 days, but it is still advisable to take things easy for the first couple of weeks and avoid heavy lifting and strenuous exercise.
The result develops gradually, and the final result is typically seen after 3–6 months, once all the swelling has subsided. You will be followed up with a check-up appointment after 3-6 months.
Your journey begins with a personal consultation. Here we go through your wishes and options, so that together we can create a treatment plan that feels right for you.
