A Brazilian Butt Lift combines liposuction and fat grafting to create more fullness, softer contours and better balance in the body's proportions. The treatment uses your own fat and can create a natural, harmonious result.
At Caolín, the treatment is always tailored to you, with a focus on your shape, your proportions and your wishes.
At a glance
A Brazilian Butt Lift – also known as a BBL – is one of the most sought-after operations in the world. It is a surgical procedure that combines liposuction with fat grafting to shape and lift the buttocks. Fat tissue is taken from unwanted fat deposits elsewhere on the body and injected where more volume and a better shape are wanted. Typical donor areas are: the abdomen, flanks, thighs or back. The fat is then purified and processed, so that only the most viable fat cells are injected. The fat is injected precisely in the places in the buttocks that create the desired shape and volume.
It is important to understand that a BBL is not simply about making the buttocks bigger. It is just as much about shaping the overall contour and creating a clearly defined waist, rounded and lifted buttocks and a harmonious transition to the thighs. Liposuction of the donor areas is an integral part of the overall result, and many of our patients choose 360-degree liposuction to achieve the body shape they want.
A BBL can also be carried out as a smaller procedure, where the focus is on improving the shape of the buttocks without increasing their size. This is relevant for women with so-called “hip dips”. Here the fat is grafted more into the hip area itself, so that the buttocks achieve an attractive curve.
At Caolín, a BBL is performed by surgeons with documented experience and a high degree of technical precision, and we always work from the latest knowledge on safe injection techniques.
To be able to carry out the procedure, it is a precondition that you have enough fat available on the body. Very slim patients may have limited options.
If you want more volume and a rounder shape in the buttocks, which cannot be achieved with exercise alone.
With flat or asymmetrical buttocks that do not harmonise with the rest of the body's proportions.
With a loss of firmness and contour in the buttocks after weight loss, pregnancy or ageing.
If you want a more defined waist and a more pronounced S-shaped contour, which can be achieved by combining liposuction and fat grafting.
Historically, a BBL has had a higher complication rate than other cosmetic procedures. The primary risk is fat embolism, a serious and potentially life-threatening condition that occurs if fat is injected into or near the deep blood vessels in the buttocks. In recent years, the international plastic surgery community has worked intensively to map and eliminate this risk, so that with the right technique a BBL can now be carried out safely and responsibly.
At Caolín, a BBL is performed exclusively using the safety protocols recommended by international professional bodies. The fat is therefore injected only into the subcutaneous layers and never into or beneath the muscle. We use large, blunt cannulas, which reduce the risk of vascular injury, and the surgeon uses an ultrasound scanner throughout to assess the depth of injection and the placement of the fat tissue.

Good to know
Here you'll find the whole process step by step — from consultation to your first weeks at home. If there's anything you can't find an answer to, bring it along to your consultation.
A BBL is a complex procedure, and it is important to have a realistic understanding of the risks. The most relevant complications are infection, uneven distribution of fat, asymmetry and limited survival of the grafted fat cells. Choosing an experienced surgeon, the correct injection technique and the use of ultrasound during the surgery are the most important factors for a safe procedure and a good, lasting result.
We advise against a BBL with a very low BMI, with certain health conditions and where expectations of the result are unrealistic.
Every treatment at Caolín starts with a thorough, no-obligation consultation, where the surgeon assesses your anatomy, your fat distribution and the options your body allows. We are honest about what the procedure can give you – and what it cannot.
You are welcome to bring someone with you, and you can contact us with questions at any point along the way.
Yes. With modern safety protocols - including injection exclusively into the subcutaneous layers, the use of large, blunt cannulas and the use of ultrasound during the surgery, the risk of serious complications is very small. At Caolín, we follow the international guidelines and only perform a BBL using documented safety techniques.
After the surgery, it is normal to have swelling, bruising, fluid build-up and in some cases temporary numbness in the treated areas. The healing process is individual, but you can expect bruising and swelling to disappear gradually after about 10-14 days.
To support good healing, you are fitted with compression garments, which you need to wear for the first 6 weeks after the surgery.
It depends on how much fat is available and on what your anatomy allows. At Caolín, a BBL is not primarily about size, but about shape and contour. Many patients want a natural, proportionate result rather than a dramatic increase. This is discussed and planned specifically at your consultation.
Typically 50–70 % of the injected fat integrates permanently. The part that does not survive is harmlessly reabsorbed by the body. The surgeon takes this into account by injecting a sufficient amount. The fat that survives is permanent and behaves like the rest of the body's fat.
To carry out a BBL, you need to have enough fat available in the donor areas. Very slim patients may have limited options. This is assessed individually at your consultation – and we are honest if your anatomy is not suited to the procedure.
The grafted fat cells are fragile in the first weeks, while they establish a new blood supply. Direct pressure compresses the cells and reduces their survival rate significantly. By avoiding sitting directly on your buttocks for 6–8 weeks, you maximise the result. A BBL cushion, which takes the pressure off the buttocks, can be used when you need to sit.
Yes – the integrated fat cells behave like the rest of the body's fat cells and are affected by significant changes in weight. A larger weight loss after a BBL will reduce the volume in the buttocks. We recommend having a stable weight before and after the procedure.
Yes – and it is a common and logical combination. The tummy tuck removes excess skin and tightens the abdomen, while the BBL shapes and lifts the buttocks. Together they give a striking transformation of the body's overall contour.
The swelling subsides gradually over the first few months, and the grafted fat stabilises. The final result – with the fat fully integrated and all the swelling gone – is visible after 3–6 months.
The process
The process starts with a thorough consultation, where you meet your plastic surgeon in calm, reassuring surroundings. Here we take the time to understand what you want for the shape of your buttocks, which areas you would like the fat taken from, and your expectations of the result.
Together we assess your body shape, your fat distribution and the quality of your skin, as well as how much fat can realistically be removed and transferred to create a harmonious, natural result. We also talk about which areas are best suited to liposuction (for example the abdomen, waist or back), so that the overall result is beautiful and well proportioned. It is important to us that you feel reassured, well informed and met with both expertise and understanding before you make your decision.
Once your treatment plan is in place, we guide you thoroughly through all the preparations leading up to the day of surgery. You will receive information on fasting, medication, dietary supplements and smoking, as these factors can affect safety, wound healing and the final result.
We also recommend preparing for your first period at home in advance – including a cushion or BBL cushion to take the pressure off your buttocks – so you can give your body the rest it needs to recover after the surgery. On the day of surgery, you arrive fasting and in comfortable clothes, and our team makes sure you feel reassured and well prepared before the treatment begins.
The surgery is performed under general anaesthetic and typically takes between two and three hours, depending on the extent. Before the surgery, the surgeon goes through the plan with you one last time and marks the areas to be treated.
During the surgery, fat is carefully removed from the selected areas using thin cannulas, after which it is purified and prepared before being injected precisely into the buttocks to create the desired shape and fullness. When the surgery is complete, the small incisions are closed, and you are fitted with a supportive compression garment that supports both the liposuctioned areas and the result in the buttocks.
After the surgery, both the donor areas and the buttocks will typically be swollen and tender for the first couple of weeks. It is important to avoid sitting directly on your buttocks for a period (typically 2-3 weeks), as this can affect the survival of the grafted fat – we guide you thoroughly in how best to take the pressure off the area in everyday life. Most people can return to lighter daily activities after a few days, while more physically demanding activity and exercise should wait 6-8 weeks.
The final result appears gradually, as some of the grafted fat is naturally absorbed by the body over the first few months, while the rest remains permanently. The final, stable result can therefore only be assessed after 3-6 months.
Your journey begins with a personal consultation. Here we go through your wishes and options, so that together we can create a treatment plan that feels right for you.
